Insurance
A Physical Therapist can evaluate and treat you without a physician's referral in Kentucky; however some insurance companies still require a physician's referral to reimburse for physical therapy services. Call our office to find out if your insurance company requires a physician's referral.
As a courtesy to our patients, we will contact your insurance company to verify benefits prior to making your first appointment. Should there be a discrepancy on benefit details reported, we encourage patients to also contact their carrier to be aware of their covered and non-covered rehab benefits. Here is a partial list of the insurances we accept:

Patient responsibilities include:
| 1. | Current medical prescription for PT, if required. |
| 2. | Payments of deductible. |
| 3. | Co-insurance or co-payment per visit. |
We accept Visa, MasterCard, Discover, cash and personal checks. A patient payment plan is also available upon request.
If you are not using insurance, the fee for the first visit is $110 for initial consultation, evaluation, assessment and treatment by a licensed physical therapist. Charges for each visit thereafter will be discussed with each patient individually. Payment is due at the time service is provided.
